    Acquired periungual fibrokeratoma: surgery treatment

    Acquired periungual fibrokeratoma is a benign fibroepitelial tumor, relatively common, but with minor incidence at the foot. It is characterized by presenting a fleshy mass covered with hyperpigmented skin and with hyperqueratosis in its distal portion, whose consistence is strong and "indurada" with a size that varies from 5 to 10 mm in length. Sometimes it can be painful after minor trauma or it can even bleed. In that case, surgical treatment can be suggested, as in the case reported. We present the case of a 31 years old patient, who comes to the office due to a painful isolated periungeal fibroma at the level of the third right toe, being recommended the surgical excision of the mass. Blood analysis and dorsolateral and dorsoplantar x-rays are required, to make sure that there´s no evidence of underlying bony pathology. The surgical technique has been carried out, under local anaesthesia making an elliptical incision following the lines of tension of the skin, and removing the fibroma. Sutures were necessary to heal the wound. After removing the mass, a biopsy was carried out and sent to the pathological anatomy department to ensure the diagnosis. The postoperative period was painless and the appearance of the toe was excellent. After one year follow-up, nor recurrences or associated problems appeared. We conclude that acquired periungual fibrokeratoma represents a well defined clinically and histologically benign tumor, easily diagnosed.     In the medical environment, the clinical study of the most basic vital signs of a patient represents the simplest and most effective way to detect and monitor health problems. There are many diseases that can be diagnosed and controlled through regular monitoring of these medical data. The purpose of this study is to develop a monitoring and tracking system for the various vital signs of a patient. In particular, this work focuses on the design of a multi-agent architecture composed of virtual organizations with capabilities to integrate different medical sensors on an open, low-cost hardware platform. This system integrates hardware and software elements needed for the routine measurement of vital signs, performed by the patient or caregiver without having to go to a medical center

    Highly hydrophobic organic coatings based on organopolysilazanes and silica nanoparticles: evaluation of environmental degradation

    Hydrophobic coatings have potential applications in various fields, including for corrosion and weathering protection. In this study, we investigated the use of organopolysilazanes (OPSZs) combined with hydrophobic nanoparticles (NPs) on steel as a substrate to obtain hydrophobic coatings. The coatings were characterized using various techniques, and their hydrophobic properties and corrosion and weathering resistance were evaluated under near-shore marine conditions with high salinity, humidity and UV radiation. Our results show that the coatings exhibited excellent hydrophobic properties and significantly improved corrosion and weathering resistance compared to an uncoated steel and a pristine polymer. These findings suggest that the developed coatings have the potential to provide protection against corrosion for atmospheric and splash exposures in marine environments.This research was funded by the Ministry of Science, Innovation and Universities of Spain, through the Torres Quevedo Program, grant number PTQ2018-009743

    Increasing the Intensity over Time of an Electric-Assist Bike Based on the User and Route: The Bike Becomes the Gym

    [EN]Nowadays, many citizens have busy days that make finding time for physical activity difficult. Thus, it is important to provide citizens with tools that allow them to introduce physical activity into their lives as part of the day’s routine. This article proposes an app for an electric pedal-assist-system (PAS) bicycle that increases the pedaling intensity so the bicyclist can achieve higher and higher levels of physical activity. The app includes personalized assist levels that have been adapted to the user’s strength/ability and a profile of the route, segmented according to its slopes. Additionally, a social component motivates interaction and competition between users based on a scoring system that shows the level of their performances. To test the training module, a case study in three different European countries lasted four months and included nine people who traveled 551 routes.     Repair of Mitral Prolapse: Comparison of Thoracoscopic Minimally-invasive and Conventional Approaches

    Objectives: Surgical repair remains the best treatment for severe primary mitral regurgitation (MR). Minimally invasive mitral valve surgery is being increasingly performed, but there is a lack of solid evidence comparing thoracoscopic with conventional surgery. Our objective was to compare outcomes of both approaches for repair of leaflet prolapse. Methods: All consecutive patients undergoing surgery for severe MR due to mitral prolapse from 2012 to 2020 were evaluated according to the approach used. Freedom from mortality, reoperation and recurrent severe MR were evaluated by Kaplan-Meier method. Differences in baseline characteristics were adjusted with propensity score-matched analysis (1:1, nearest neighbour). Results: Three hundred patients met inclusion criteria and were divided into thoracoscopic (N = 188) and conventional (sternotomy; N = 112) groups. Unmatched patients in the thoracoscopic group were younger and had lower body mass index, New York Heart Association class and EuroSCORE II preoperatively. After matching, thoracoscopic group presented significantly shorter mechanical ventilation (9 vs 15 h), shorter intensive care unit stay (41 vs 65 h) and higher postoperative haemoglobin levels (11 vs 10.2 mg/dl) despite longer bypass and cross-clamp times (+30 and +17 min). There were no differences in mortality or MR grade at discharge between groups nor differences in survival, repair failures and reinterventions during follow-up. Conclusions: Minimally invasive mitral repair can be performed in the majority of patients with mitral prolapse, without compromising outcomes, repair rate or durability, while providing shorter mechanical ventilation and intensive care unit stay and less blood loss
